    Attention, awareness, and understanding of the flight crew are a critical contributor to safety and the flight deck plays a critical role in supporting these cognitive functions. Changes to the flight deck need to be evaluated for whether the changed device provides adequate support for these functions. This report describes a set of diverse evaluation methods. The report recommends designing the interface-evaluation to span the phases of the device development, from early to late, and it provides methods appropriate at each phase. It describes the various ways in which an interface or interface component can fail to support awareness as potential issues to be assessed in evaluation. It summarizes appropriate methods to evaluate different issues concerning inadequate support for these functions, throughout the phases of development

    Comparison of COPD primary care in England, Scotland, Wales, and Northern Ireland

    Currently the National Asthma and COPD audit programme (NACAP) only undertakes audit of COPD primary care in Wales due to its near complete data coverage. We aimed to determine if the quality of COPD primary care in the other UK nations is comparable with Wales. We found that English, Scottish, and Northern Irish practices were significantly worse than Welsh practices at recording coded lung function parameters used in COPD diagnosis (ORs: 0.51 [0.43–0.59], 0.29 [0.23–0.36], 0.42 [0.31–0.58], respectively) and referring appropriate patients for pulmonary rehabilitation (ORs: 0.10 [0.09–0.11], 0.12 [0.11–0.14], 0.22 [0.19–0.25], respectively). Completing national audits of primary care in Wales only may have led to improvements in care, or at least improvements in the recording of care in Wales that are not occurring elsewhere in the UK. This highlights the potential importance of audit in improving care quality and accurate recording of that care

    Spirometry parameters used to define small airways obstruction in population-based studies: Systematic review

    Background The assessment of small airways obstruction (SAO) using spirometry is practiced in population-based studies. However, it is not clear what are the most used parameters and cut-offs to define abnormal results. Methods We searched three databases (Medline, Web of Science, Google Scholar) for population-based studies, published by 1 May 2021, that used spirometry parameters to identify SAO and/or provided criteria for defining SAO. We systematically reviewed these studies and summarised evidence to determine the most widely used spirometry parameter and criteria for defining SAO. In addition, we extracted prevalence estimates and identified associated risk factors. To estimate a pooled prevalence of SAO, we conducted a meta-analysis and explored heterogeneity across studies using meta regression. Results Twenty-five studies used spirometry to identify SAO. The most widely utilised parameter (15 studies) was FEF25–75, either alone or in combination with other measurements. Ten studies provided criteria for the definition of SAO, of which percent predicted cut-offs were the most common (5 studies). However, there was no agreement on which cut-off value to use. Prevalence of SAO ranged from 7.5% to 45.9%. As a result of high heterogeneity across studies (I2 = 99.3%), explained by choice of spirometry parameter and WHO region, we do not present a pooled prevalence estimate. Conclusion There is a lack of consensus regarding the best spirometry parameter or defining criteria for identification of SAO. The value of continuing to measure SAO using spirometry is unclear without further research using large longitudinal data. PROSPERO registration number CRD4202125020

    Airplane Capabilities: Translating Non-Normal Information for Operational Decision-Making

    We consider how a jet transport airplane interface supports the flight crew in managing airplane system failures (or non-normals) for continued safe flight and landing. The existing state of the art starts with a list of airplane system component failures and asks the flight crew to determine, with the help of non-normal procedures, the operational consequences of those failures. As airplane systems become more complex and interconnected, the flight crew's ability to determine operational consequences will become inadequate. We describe an approach that attempts to translate airplane system failures directly into airplane "capabilities," which is a set of basic airplane functions, such as the ability to stop after landing. This paper describes the overall framework for supporting flight crews in operational decision making and the initial efforts to develop a language and display concepts

    The influence of wave exposure on coral community development on man-made breakwater reefs, with a comparison to a natural reef

    Breakwaters dominate shorelines in many coastal urban areas, providing substantial hard-bottom habitat upon which diverse and abundant reef communities develop. In recognition of their potential ecological and economic importance, there is increasing interest in understanding how design features can influence community development. We investigated the influence of wave exposure on breakwater coral communities in Dubai, United Arab Emirates. Coral community composition, cover, size structure, recruitment, mortality, and growth rates were compared quarterly between two windward and two leeward breakwater sites for 1 yr to explore the influence of wave exposure on coral community development. Comparisons also were made with a natural coral reef to gain an understanding of how community structure and dynamics compare between these habitats. Benthic and water column sediment particle sizes were also analyzed. Leeward breakwaters contained a low-cover coral community dominated by small colonies with high mortality compared with windward breakwaters and the natural reef. Windward breakwater coral communities had comparable recruitment, mortality, and growth rates as the natural reef. Fine sediments ( 125 μm), likely as a result of differences in wave action among reef types. Overall, these results suggest that leeward breakwaters represent sub-optimal habitats for coral community development. However, with appropriate design, breakwaters can develop diverse and abundant coral communities with comparable coral cover, demographics, and growth rates to those on the natural reef in Dubai. © 2010 Rosenstiel School of Marine and Atmospheric Science of the University of Miami

    The Role of Alerting System Failures in Loss of Control Accidents CAST SE-210 Output 2

    This report is part of a series of reports that address flight deck design and evaluation, written as a response to loss of control accidents. In particular, this activity is directed at failures in airplane state awareness in which the pilot loses awareness of the airplane's energy state or attitude and enters an upset condition. In a report by the Commercial Aviation Safety Team, an analysis of accidents and incidents related to loss of airplane state awareness determined that hazard alerting was not effective in producing the appropriate pilot response to a hazard (CAST, 2014). In the current report, we take a detailed look at 28 airplane state awareness accidents and incidents to determine how well the hazard alerting worked. We describe a five-step integrated alerting-to-recovery sequence that prescribes how hazard alerting should lead to effective flight crew actions for managing the hazard. Then, for each hazard in each of the 28 events, we determine if that sequence failed and, if so, how it failed. The results show that there was an alerting failure in every one of the 28 safety events, and that the most frequent failure (20/28) was tied to the flight crew not orienting to (not being aware of) the hazard. The discussion section summarizes findings and identifies alerting issues that are being addressed and issues that are not currently being addressed. We identify a few recent upgrades that have addressed certain alerting failures. Two of these upgrades address alerting design, but one response to the safety events is to upgrade training for approach to stall and stall recovery. We also describe issues that are not being addressed adequately: better alert integration for flight path management types of hazards, airplanes in the fleet that do not meet the current alerting regulations, a lack of innovation for addressing cases of channelized attention, and existing vulnerabilities in managing data validity

    In 2014, the Commercial Aviation Safety Team produced a report describing the results of an analysis intended to understand and mitigate airplane incidents and accidents associated with flight crew loss of attitude or energy state awareness. That report described several "Safety Enhancements" including a new category of "Research Safety Enhancements". This paper focuses on Safety Enhancement (SE) 210 Output 2, investigating improvements in design methods and guidelines to "assess flight crew performance in situations associated with loss of energy and/or attitude state awareness"

    Considerations for the Use of Remote Gaze Tracking to Assess Behavior in Flight Simulators

    Complex user interfaces (such as those found in an aircraft cockpit) may be designed from first principles, but inevitably must be evaluated with real users. User gaze data can provide valuable information that can help to interpret other actions that change the state of the system. However, care must be taken to ensure that any conclusions drawn from gaze data are well supported. Through a combination of empirical and simulated data, we identify several considerations and potential pitfalls when measuring gaze behavior in high-fidelity simulators. We show that physical layout, behavioral differences, and noise levels can all substantially alter the quality of fit for algorithms that segment gaze measurements into individual fixations. We provide guidelines to help investigators ensure that conclusions drawn from gaze tracking data are not artifactual consequences of data quality or analysis techniques

    Occupational exposures and small airways obstruction in the UK Biobank Cohort

    Background Small airways obstruction (SAO) is a key feature of both Chronic Obstructive Pulmonary Disease and asthma, which have been associated with workplace exposures. Whether SAO, which may occur early in the development of obstructive lung disease and without symptoms, also associates with occupational exposures is unknown. Methods Using UK Biobank data, we derived measurements of SAO from the 65,145 participants with high quality spirometry and lifetime occupational histories. The ALOHA+ Job Exposure Matrix was used to assign lifetime occupational exposures to each participant. The association between SAO and lifetime occupational exposures was evaluated using a logistic regression model adjusted for potential confounders. A second logistic regression model was run to also account for potential co-exposures. Results SAO was present in varying proportions of the population depending on definition used: 5.6% (FEF25–75<LLN)and 21.4% (FEV3/FEV6<LLN). After adjustment for confounders and co-exposures, people in the highest category of exposure to pesticides were significantly more likely to have SAO (FEV3/FEV6<LLN: OR 1.24, 95%CI 1.06–1.44). The association between pesticides and SAO showed an exposure-response pattern. SAO was also less likely among people in the highest exposure categories of aromatic solvents (FEV3/FEV6<LLN: OR 0.85, 95%CI 0.73–0.99) and metals (FEV3/FEV6<LLN: OR 0.77, 95%CI 0.62–0.94). Conclusion Our findings suggest that occupational exposure to pesticides play a role in the SAO. However, further work is needed to determine causality, and identify the specific component(s) responsible and the underlying mechanisms involved
